Applications for school registration are not available online and are triplicate forms available at the office between the hours of 8:00-5:00 during the school week M-F. While you must live within the area designated by the LAUSD to be eligible to apply, there is a lottery which allows people in from outside the area.

Please print the attached Information Request Form PDF file and complete it for your child/children. This form is in addition to the LAUSD-required emergency cards and covers all of the data required for Connect-Ed, School Directory, Class Rosters, and Business-to-Business Directory. We understand how labor intensive the beginning of the year paperwork can be and appreciate your completing all of the required forms by the first Monday following Labor Day.

In an effort to conserve your time, one form can be completed for the entire family with space on the back to document additional students, parents, guardians, or business listings. Please note that the parent or guardian listed as Parent #1 is considered the Primary Contact and will be targeted to receive the various updates sent throughout the year via email, personal phone call or Connect-Ed telephone bulletin.
